I. Project Name: Bidding for medical service of Iraq Halfaya Project

2. Service Content: 2 physicians, medicine procurement, medical equipment calibration, and cross - border medical transportation.

3. Project Location: Iraq Missan Halfaya Project Site

4. Service Period: From Aug. 1, 2025, to Feb. 28, 2026

II. Technical Requirements  

1. Personnel Requirements:  

   - 2 Chinese physicians  

2. Technical Specifications:  

   - Possess a physician qualification certificate;

- First-aid certificate, with over 3 years of overseas work experience.

-Be physically and mentally healthy, with a valid health certificate

3. Deliverables:  

   - Safeguard the health of Chinese and international staff on site; issue warnings for seasonal and epidemic diseases;

-Conduct weekly health and sanitation checks of the base's kitchen and storeroom areas; organize monthly health checks for drilling and workover rig personnel, and inspect medical rooms, kitchens, and warehouses;

-Provide employee first - aid training; track key employees' health indicators weekly; ensure new arrivals undergo health checks within 2 weeks in Iraq and supervise the uploading of results to the overseas risk warning APP;

- Guide employees with BMI over standard to reduce weight scientifically; properly handle employees' sudden illnesses and injuries; update and drill the cross - border medical transportation procedure.

4. Warranty Terms:  

   -. Meet the on - site medical service standards of PetroChina Halfaya and project.

III. Bidder Qualification Requirements  

(A) Basic Qualifications  

1. Independent legal entity with general taxpayer status.  

2. Business license covering for providing medical services.  

3. No major legal violations in the past two years.  

4. Sound financial status (submit audited financial reports for the last two years).  

(B) Professional Qualifications  

1. Qualifications for providing medicine procurement, medical equipment calibration, and cross - border transportation (or equivalent proof).  

2. Possess ISO qualifications (project proof required)..  

(C) Other Requirements  

1. Joint bids are not accepted.  

2. Must have USD payment capability (provide proof).  

IV. Pre-Qualification  

1. Application Period: May 22–26, 2025 (Iraq time, deadline: 24:00).  

2. Required Documents:  

   - Unified Social Credit Code business license.

   - Proof of USD account.  

   - Medical service qualifications (or equivalent proof).

   - No major legal violations in the past two years

- Audited financial reports for the last two years.  

   - ISO qualifications (project proof required).

   - Bidder qualification declaration (format optional).

V. Bidding Process  

If fewer than 3 qualified bidders apply, the process will proceed upon approval from higher authorities. If more than 3 qualified bidders apply, the bid documents will be issued via email.

This tender is subject to Chinese and Iraqi laws. The bid issuer reserves the right to cancel or re-initiate the bidding process.
